Description

In the 21st century, we are within a continuous revolution in technology applied to surgical techniques. Laparoscopic and robotic surgery are a good example. The development of these surgical techniques, as well as their improvement, require training and learning that must be able to be carried out outside the real operating theatre and the daily scenario of our patients. Pelvic trainers, simulators and human specimens are models that facilitate learning, but the animal model allows the surgeon to obtain an excellent approximation to a living, palpable organism, where he can learn, maintain and improve endoscopic surgery and carry out specific training in each of the different surgical specialities. This course will have a team of expert teachers in laparoscopic surgery, who will teach the students the theoretical bases and the different exercises in the porcine model, using advanced surgical material in laparoscopic surgery.

Goals

To improve skills in gynaecological laparoscopic surgery, through practice on pelvic trainers and animal models, which will be adapted to the level of the students in a progressive manner, with a maximum of 4 levels:

  • Level 1: theory session + pelvitrainer

Ergonomics, surgical material, technical exercises, continuous and discontinuous suturing techniques with intracorporeal and extracorporeal knots.

  • Level 2: in vivo surgery

Abdominal wall incisions, pneumoperitoneum, open and closed entry techniques (safe umbilical entry, Palmer’s stitch), salpinguectomy, oophorectomy, morcellation of organic material, etc.

  • Level 3: in vivo surgery

Hysterectomy, prevention and repair of bladder, ureteral and intestinal injuries, use of haemostatic material.

  • Level 4: in vivo surgery

Pelvic lymphadenectomy, para-aortic lymphadenectomy, colposacropexy, prevention and repair of vascular lesions.

Students

Junior specialists in Obstetrics and Gynaecology, who wish to develop an apprenticeship in general and advanced laparoscopic surgery.

Course details

The learning method is theoretical-practical and consists of the tutored students carrying out the programmed exercises and surgical interventions, alternating as surgeon and assistant. All the activity will be carried out with maximum interaction between the student and the tutor, in order to resolve any doubts.

Theoretical classes, pelvic trainers and surgical stations on an animal model will be used to achieve the objectives. The students will have an individual pelvitrainer and a surgical station for every 2 students, where they will actively participate in the surgical field, as surgeons and as surgical assistants, with the help of a tutor for each station.

Each student will be assessed and tutored in a personalised manner by specialists in gynaecological endoscopic surgery. At the end of the experimental animal training period, the student will receive knowledge and training in:

  • Laparoscopic surgical material and equipment.
  • Dissection and endoscopic electrocoagulation techniques.
  • Continuous and discontinuous, intracorporeal and extracorporeal endoscopic suturing.
  • Vertical, horizontal and lateral suturing, with both hands, as dominant and secondary.
  • Pneumoperitoneum with open and closed technique.
  • Total hysterectomy with bilateral annexectomy.
  • Bladder and ureteral dissection.
  • Pelvic and para-aortic lymphadenectomy.
  • Mechanical morcellation.
  • Laparoscopic management of complications: bladder, intestinal and vascular lesions.
